1. Acceptance of These Terms

These Terms and Conditions apply to this page and any related pages operated by Bloomington Family Dental ("we," "us," "our"), located at 9820 Nicollet Ave S, Bloomington, MN 55420. By using this page, you agree to these terms. If you do not agree, please do not use this page.

2. Not Dental Advice

The content on this page is for general information only. It is not dental or medical advice, and it is not a substitute for an in-person exam by a licensed dentist. Using this page or calling our office does not create a dentist-patient relationship. A dentist-patient relationship begins only after you are seen in our office. If you have a dental emergency, call our office at 952-295-5053. For a medical emergency, call 911.

3. Results Vary

Treatment outcomes depend on each patient's oral health, history and follow-through with care instructions. We do not guarantee any specific result. Photos, reviews and testimonials reflect individual experiences and are not a promise of similar results.

4. Treatment Recommendations and Second Opinions

All treatment recommendations, including for sleep apnea and TMJ/TMD, are made only after an in-person exam and consultation with a licensed dentist. Not every patient is a candidate for every treatment.
Sleep apnea care at our office may be coordinated with your physician. A diagnosis of sleep apnea is made by a qualified medical provider. [CONFIRM WITH PRACTICE]
Second opinions are based on our own exam and findings. They are not a review or judgment of another provider's care, and our recommendations may differ from those you received elsewhere.
Same-day emergency appointments are offered whenever possible and depend on availability.

5. Insurance, Membership Plans and Financing

Insurance: We accept most private and local dental insurance plans. Coverage varies by plan, and a plan name appearing on this page does not guarantee coverage for any specific service. You are responsible for any amount your plan does not cover. We do not accept Medicaid or other state-funded dental plans.
Membership plans: Our Adult ($400 per year), Child (18 and under, $300 per year) and Periodontal ($700 per year) membership plans are 12-month discount programs, not dental insurance. They are valid only at Bloomington Family Dental. Membership fees are non-refundable, benefits are active only while your membership is current, and discounts do not apply to services referred to outside specialists. The Periodontal plan does not include scaling and root planing. Plan details and pricing are subject to change, and full terms are available at our front desk.
Financing: Financing and payment plan options are subject to approval and to the terms of the provider offering them.
Any treatment will be explained and priced before it begins.

6. Calls and Text Messages (SMS Terms)

When you call us, our phone system may log your phone number and the time and length of your call so we can return missed calls and measure how our advertising performs. [IF CALLS ARE RECORDED: Calls may be recorded for quality and training purposes, and you will be told at the start of the call.]
SMS program: If you consent to receive text messages from Bloomington Family Dental, you agree to receive customer care messages from us, such as replies to your inquiry, appointment reminders and follow-ups.

Consent to receive text messages is not a condition of any purchase or service.
Carriers are not liable for delayed or undelivered messages.
No mobile opt-in consent or phone numbers will be shared with third parties or affiliates for marketing or promotional purposes.

Any awards or recognition shown on this page were given by third parties and reflect their own criteria. They are not a guarantee of results.
